WHAT ARE THE OFFICE HOURS?

Our office is open Monday-Thursday 7:30 am - 4:00 pm. Office closed for lunch 12:30 pm-1:00 pm. We are closed Friday/Saturday/Sunday.

WHAT HEALTH INSURANCE PLANS DO YOU ACCEPT?

We accept most health insurance plans. Please see our Insurance page or call our office (850)785-8246

IF I AM SICK, DO I NEED AN APPOINTMENT OR CAN I WALK IN?

Patients are seen by appointment only, however we can schedule same-day appointments if available. Urgent appointments will be scheduled based on the type and severity of the problem. If you have a true medical emergency, call 911 or visit ER/Urgent care facility.

HOW DO I MAKE AN APPOINTMENT?

You can make an appointment by calling 850-785-8246, online through the Patient Portal, or via email practice@lubinmd.com

HOW DO I CANCEL/CONFIRM MY APPOINTMENT?

Call 850-785-8246 ext.1, email us, or log on to Patient Portal.   Any cancellations, rescheduled or missed appointments with less than a 24 hour notice, will be assessed a $100.00 fee.

DO YOU DO BLOOD WORK AT YOUR OFFICE? 

We offer lab draw on site on Wednesday morning only.  You can have lab draws done at any lab facility of your choice/your insurance preferred laboratory. If blood work is needed, you will be given a lab order. The lab results will come back to our office for your physician to review.

 

HOW WILL I BE NOTIFIED OF TEST RESULTS?

Patients are typically given a follow up appointment at the time tests/labs are ordered. Results will not be given over the phone. 

 

HOW DO I GET MY PRESCRIPTION REFILLED? 

Contact your pharmacy at least week before you run out of your medication and have your pharmacy send a refill request to our office. If an appointment is needed prior to your medication refill your pharmacist will notify you. It is important to keep your scheduled appointment to ensure that you receive timely refills. Refills can only be authorized on medication prescribed by providers from our office. We will not refill medications prescribed by other providers. Please allow 72 hours for all RX refills.

 HOW CAN I ACCESS MY MEDICAL RECORDS?

This practice believes in staying current with the latest technology, which in turn helps us to take better care of you. We offer a  Patient Portal for electronic medical records and communication with your physician and staff. We also provide a paper copy of your lab/test results at the time of your follow up office visit.

HOW CAN I GET A PAPER COPY OF MY MEDICAL RECORDS?

Paper copies of medical records must be requested in writing. To ensure your privacy, a form for release of medical information must be completed prior to receipt any medical records. All patients can request a copy of their medical records at a cost of $1.00 per page. No cost to the patients for medical records shared with Medical Practices. The law allows Medical Offices 30 days to complete requests for records. However, our medical staff puts forth every effort to respond to these requests in a timely manner.
